Leuprolide, what the science actually shows

Leuprolide is a synthetic GnRH agonist FDA-approved for prostate cancer, endometriosis, precocious puberty, and uterine fibroids. Evidence base: extensive clinical data across multiple approved indications.

A synthetic GnRH agonist that paradoxically suppresses sex hormone production through receptor desensitization. FDA-approved for multiple indications. One of the most commercially successful peptide drugs ever.

What leuprolide is

Synthetic nonapeptide analogue of natural GnRH, 15-100x more potent and resistant to degradation. Marketed as Lupron since 1985. Available in daily to 6-month depot formulations. One of the first blockbuster peptide drugs.

Mechanism, in plain English

Natural GnRH pulses signal the pituitary to produce LH and FSH. Continuous GnRH (instead of pulses) first causes a surge ('flare'), then desensitizes and downregulates receptors. After 2-4 weeks: LH, FSH, testosterone, and estrogen drop to castrate/menopausal levels. Fully reversible when stopped.

What the evidence shows

Prostate cancer: standard of care for advanced disease, reduces testosterone to castrate levels in 95%+. Endometriosis: reduces pain 60-80%. Uterine fibroids: shrinks 30-60% over 3-6 months. Precocious puberty: halts premature development, preserves adult height. IVF: prevents premature LH surge. Decades of safety data.

FDA status

FDA-approved for prostate cancer, endometriosis, uterine fibroids, central precocious puberty, IVF.

Common synonyms

Safety and adverse-event landscape

Initial hormone flare (1-2 weeks). Hot flashes (common), bone density loss with long-term use, mood changes. Cardiovascular signals in long-term androgen deprivation.

Frequently asked questions

Hormonal effects reversible in most cases. Bone density loss partially reversible. Fertility typically returns within months.
Flare from continuous stimulation before desensitization (1-2 weeks). GnRH antagonists avoid this.
Yes. Synthetic 9-amino-acid GnRH analogue. One of the most successful peptide drugs in history.
